We’re a service-delivery agency for New York’s Early Intervention program. Once your child has been evaluated by your county and has an IFSP, you can choose Achievable Milestones to provide their therapy — at home, on your schedule, with credentialed clinicians who care.
How it works
Where we fit in the EI process.
Early Intervention in New York has multiple steps, handled by different organizations. Here's the part we play:
You + your county
Referral and evaluation
You contact your county's EI program (NYC 311, Nassau Health Dept., or Westchester Health Dept.). An Initial Service Coordinator is assigned and a free evaluation is scheduled. We don't do this part — your county does.
You + your county
IFSP meeting
If your child qualifies, you and the team write an Individualized Family Service Plan (IFSP) — the document that says what services your child will receive. This is also handled by your county and an evaluation team.
You choose
You pick a service-delivery agency
You get to choose which agency provides your child's services. This is where we come in. Your service coordinator can refer you to us, or you can request us specifically.
Us
We deliver the services
We send credentialed therapists to your home (or daycare) for the sessions in your IFSP. Speech, OT, PT, special education, family training — whatever your IFSP says. Our work begins here.

Once you have an IFSP, these are the services we can deliver. All sessions happen in your child's natural environment — your home, daycare, or wherever they're most themselves.
Speech & language therapy
Speech-Language Pathologists (SLPs)
For children with delays in expressive language, receptive language, articulation, or social communication. Includes feeding therapy when called for by your IFSP.
Occupational therapy
Occupational Therapists (OTs)
Supporting fine motor skills, sensory processing, self-care, and the everyday skills children use to participate in family life.
Physical therapy
Physical Therapists (PTs)
Helping children build strength, coordination, balance, and gross motor milestones — from rolling and crawling to walking and running.
Special instruction
Special Educators / SEITs
Individualized teaching tailored to your child's developmental level, with focus on cognitive skills, play, problem-solving, and adaptive behavior.
Family training & support
All providers
Coaching for parents and caregivers — because the most powerful learning happens between sessions, in everyday moments. Every visit includes family training.
Service coordination support
In partnership with your county coordinator
Your county assigns your service coordinator. We work closely with them to keep your services running smoothly and your IFSP on track.

A real conversation first
When your service coordinator refers you to us — or when you reach out with an IFSP in hand — we don't just plug you into a system. We'll talk on the phone, hear what's going on, and walk you through what comes next.
A consistent provider
The same therapist comes to your home for every session, building real rapport with your child. No revolving doors of substitutes.
Sessions that fit your life
Mornings, evenings, mid-day naps — we work around your family's actual schedule, not the other way around.
Coaching, not just therapy
You're a full participant in every session. We teach you what we're doing and why, so you can use the same techniques in everyday moments.
Regular updates
You'll get session notes after every visit and a summary every few weeks. No mystery — you'll always know what's happening with your child's progress.

Your provider gives you their direct contact. Questions between sessions? Send a message. We'll respond.

Do you do the initial evaluation?+
No. Evaluations are handled by your county's Early Intervention program through separate evaluation agencies. Our role begins after you have an IFSP and choose us as your service-delivery agency. If you haven't started the EI process yet, contact your county directly: NYC families call 311, Nassau County families call (516) 227-8661, and Westchester families call (914) 813-5094.
How do I get my child to Achievable Milestones specifically?+
Once you have an IFSP, you can request us by name through your service coordinator. Your county is required to honor family choice on the agency that delivers services, as long as we have availability in your area. Reach out to us anytime — we'll explain the request process to your coordinator if needed.
How long does it take to start services after I have my IFSP?+
Once you've selected us and your service coordinator has confirmed the referral, we typically have your first session scheduled within 1–2 weeks. We move fast on our end.
